COMMUNITY HEALTH DIAGNOSES
The primary focus of a community health nurse is to improve the health of the community. To do this involves using demographic and epidemiological findings to assess the community’s health and diagnose its needs. This assessment is aligned with the module objective “Assignment help – Discuss the elements of practice in community/public health nursing.”
This assignment is not a paper, so there is no cover page. All of the work is completed by completing the attached template. As part of the assignment, you will complete the following tasks: typing data, an analysis of the findings with four (4) references, and community health nursing diagnoses directly on the template.
Compile a range of relevant demographic and epidemiological data found on the community assessment rubric for this assignment. Use the websites listed below, as well as other websites (you can also do a Google search to find the health data you are looking for), gather information about your county and compare it with your state and national numbers.

Analysis:
Key Risks and Concerns
The key risks and concerns based on the data gathered is that Franklin County community faces high deaths associated with cardiovascular disease and lung cancer. The data indicates that the death rate of cardiovascular disease in Franklin County per 100000 people is 174.7 being the highest cause of death followed by lung cancer at a rate of 45. The existence of the two concerns may be facilitated by other factors in the county. Some of the risk factors for cardiovascular disease and diabetes include poverty rate at 13.5% considered high in a county with unemployment rate of 4.9 percent. According to Healthy People 2030, 16 percent of Franklin County residents overall health is considered “fair” or “poor”, which is slightly lower than Ohio State and the National overall. Lung cancer is the common cause of death among Franklin County males while the most death for females is dementia. The County objective of reducing the lung cancer death, routine screening has been initiated with almost 65 percent of adults aged 50 to 75 undergoing screening in the past three years. The hospitals in the county has also enhanced emergency departments to handle cardiovascular disease
