COMMUNITY HEALTH ASSESSMENT
When it comes to getting healthier, the answer is not always to find the right medicine. Sometimes, you might need to change where you are. In the United States, many people live in places where they can’t easily get to grocery stores, medical clinics, or places to get some exercise. Due to the complexity of modern cities, trying to improve the health of a whole community may be hard. The authors write, “One big problem is that we don’t have a best practices model for place-based treatments.”
To improve the health of a community, the first step is to find out what the community values most (Sharpe, Greaney,2020). Everyone who lives there needs and benefits from the sense of community that exists there. You can learn about the people and places in a community from the way they are set up. Every one of these lives has the chance to grow or to die. For the community’s hidden histories to come out, a community health assessment is needed. With this kind of information, doctors will be able to help the people in their communities better. This report on the state of Jordan, Minnesota, is made up of information from many different places. The information came from a windshield poll of local people. Health care needs in Jordan, Minnesota, can be better understood, and the strengths, weaknesses, and unique qualities of the community can be shown more clearly.

Assessing the health of a community can tell you a lot about the community’s relationships, problems, and resources when it comes to health. Jordan, Minnesota, is a small town with a population of 5,981 and a median household income of $66,818. It is a “rural town” (Menden, et al., 2019). If I were doing a windshield survey, I would make sure to include the city hall, every school, library, hospital, church, park, recreation center, store, and shopping mall. Parks, neighborhoods, and city streets are all places in Jordan City that are relevant to what we are talking about. These things are important because of how they make people feel. All of the spaces are used often by the locals. The survey is limited because it can’t directly ask people in the community. For instance, I worry about my own safety if I have to drive slowly by to get all the information I need for the survey. From the results of the poll, I’ve come to the conclusion that this is a growing community with a healthy economy. In the city, one might see their family or business do well.
Analysis of the environment
When I was doing the windshield survey, a few questions came to mind that I’d like answered. How many young people from this area go on to college? How many people in the world are in good health? Community health care would focus on things like technology and money. Because there aren’t many people living there, I’m guessing there aren’t any modern conveniences. In Jordan, APRNs may be able to find a wide range of job opportunities. Because it is a small community, there is also a chance that medical facilities like hospitals and clinics will be too full (Shin, et al., 2019). Everyone in this town seems to know each other.
They have a system in place that works well, and they are also willing to change and grow so that they can get more business and make it easier for people to get there. The fact that people want to fill the empty spaces with new things is a strong sign. The health of a community is affected by social factors like how easy it is to get an education, a job, or medical care. Doctors and nurses who share what they know with the rest of the community could do a lot of good for the health community. Population changes are just one way that demographics can have an effect on society. When the number of people in a place goes up or down, the social environment can change.
Conclusions
In terms of data, the information gathered from the city of Jordan will be put into an Excel spreadsheet, while the other information will be shown in a Powerpoint presentation. The people I want to reach need and want to know certain things about economic growth, educational opportunities, and health care. Based on the information from different sources, this windshield analysis told me how the local area was doing. Doing a community health assessment clears the way for its regular use, which in turn clears the way for any changes that need to be made and for making decisions (Menden, et al., 2019). Everyone, from people to businesses, can broaden their view of the world. But the age of technology and the progress of society aren’t perfect. The most important thing to learn from this poll is how important education and experience are to the survival of a group. Every group needs a different set of resources to do well.
Place-based treatments can only work if the whole community is involved. This is a lot like community-based participatory research, which is a way to do research in which the people in the community decide which questions need to be answered
