I. HPI and Clinical Impression

GM is a 45-year-old African American working mother of 3 children who has been diagnosed with GDD.
She reports experiencing symptoms of depression, including feelings of sadness, hopelessness, and lack of interest in activities.
She is currently on medication and receiving psychotherapy.
Appetite and sleep are reported as good and there are no suicidal ideations
II. Psychopharmacologic Treatment

Recommendation: SSRI (Selective Serotonin Reuptake Inhibitor) such as Sertraline or Fluoxetine
Specific and therapeutic endpoints:
Achieve remission of symptoms of depression as measured by the Hamilton Depression Rating Scale (HAM-D)
Improve functioning as measured by the World Health Organization Disability Assessment Schedule 2.0 (WHODAS 2.0)
III. Psychotherapy

Recommendation:
Individual therapy: cognitive-behavioral therapy (CBT)
Family therapy: Family-Focused Therapy (FFT)
Specific therapeutic endpoints:
Improve coping skills and problem-solving abilities
Improve communication and relationship dynamics within the family
IV. Medical Management

Primary care needs:
Regular monitoring of blood pressure and weight
Screening for diabetes and other comorbid conditions
Monitoring for potential drug interactions and side effects
