Write out an implementation and evaluation plan

For this assignment, you will write out an implementation and evaluation plan for your policy change or research study proposal.
Start with the implementation plan that should detail how you will go about the change in practice. Consider the steps required to implement the plan as well as resources and budget and include key stakeholders (people that influence the change or are affected by the change). Who are the stakeholders? Where do they currently stand in terms of support of or resistance to the project? What level of support can be expected from them? Other questions to consider include: Who will be involved in the plan? How long will the change take to implement? What resources are needed for the change? What data will be collected? Are there any risks associated with the change?
Next decide what outcomes you will measure or in other words how will you know if the change is an improvement? List and discuss at least two outcome measures. The discussion should include why this measure is important as well as how and why it shows an improvement. Outcomes should be measurable.
Use websites such as the Agency for Healthcare and Research Quality and the IHI to support your discussion. Implementation and Evaluation Plan should be at least two pages, double spaced in APA format, excluding the title and reference pages.

Use any format you like for the plan, but be sure that you:
Demonstrate good overall writing skills.
Formal, scholarly writing with no first person language.
Writing is clear and concise, free of APA errors.
References and in-text citations follow APA 7th edition guidelines. Paraphrase as appropriate. Use peer-reviewed references published within the last 5-7 years.
Writing is free of grammatical and spelling errors.
Follow assignment instructions.
Plagiarism or incorrect paraphrasing will result in a minimum of a 20% deduction on an assignment. Deletion of additional points or a score of zero may also be a result of incorrect paraphrasing. Please review the APA manual on paraphrasing and online writing resources.
