While covering the ICU during your scheduled night shift, you clearly notice your coworker documenting ventilator checks without actually completing the ventilator check. You professionally confront the individual, expressing your concerns. Your coworker states, “we are too busy to get all of work done, so we have to cut corners someplace…it’s really no big deal”. You further explain that such behavior compromises patient safety and feel obligated to report the situation.
A few days later, you receive a phone call from the Risk Management department at the hospital. The Safety Officer requests that you provide an SBAR regarding the event(s) and forward to Risk Management, as well as your manager.
